In a 9 x 12 envelope should be sealed with the players name on it and turned in to the concession stand by Thursday June 11 and should have the following items in the envelope
1 Original birth certificate.( All Teams)
1 copy of birth certificate(all teams))
Proofs of Residence should have been turn in at registration, if you down loaded it at registration or turned in at registration we will have it if not you will need to provide them
3 Proofs of residence dated 2/1/25 to 2/1/26
easiest is 1.Driver license ,2. utility bill and 3.Car registration
or a school enrollment form
A check for $70 make payable to NOLL or with credit card at the concession stand. Worker will mark paid on your envelope
Medical release Forms on website or Manager will have one
All-Star Apparel shop link on our website later by Tuesday
Managers will be selected in coming days and will be contacting you either Monday or Tuesday.
